Been in a number of hotels (decent ones, too) in Latin America where hot water for the shower comes from an electric thing that the water comes out of. You start the water going, plug it in(!), wash, unplug it, then turn the water off. (I think I remember the instructions being emphatic about having the water running when the heater is on, but maybe that's just the instructions for gringos).
