We run a custom software development process, somewhere in between Agile and RUP. To document this we wanted to keep things as simple as possible, and we managed to do it. A single page document which shows a nice high level flow diagram. It has approx 16 steps, and is very easy to view.
The above one pager was captured onto our wiki, and a few noted points were added where necessary to describe it. Whats great is its actually followed by us and is used by the other departments within our company. Much better than the sometimes over documented processes that I've dealt with in the past.
