The archetypical MUD was written by Richard Bartle, yes, and it was written on the University of Essex DEC-10.
It was later run at the U of York DEC-10, and at the U of Oslo (Norway) DEC-10. It was written in a mixture of BCPL and MACRO-10. I still have my BCPL manuals lying around somewhere here.

Yes, the PDP-10s had PDP-11 console frontends for diagnostics and boot. They ran a specialized OS named RSX-20F.
Before networking became an option, it was also usual to have a number of PDP-11-based terminal server frontends.
(Newer PDP-10-based system such as the DECSYSTEM-2065 and DECSYSTEM-2020 had integrated LSI-11 CPU frontends. For us that were used to the full-size PDP-11 systems provided with systems such as the DECsystem-1099, this was kind of a letdown.)
