I memorize set of transforms on words that spits out different words.
e.g. Transform1: always replace "apple" with "orange"
Transform2: Replace the letter "e" with "eat"
Then I write down all the passwords against each site and mark which transform I have used for each i.e. Transform1 or Transform2. Since only I know what each transform means, even if someone sees my list of passwords, they can't do anything about it. I don't have to refer to the table of passwords for all the commonly used passwords coz my fingers remember it somehow. So this is working out pretty well so far.
