You have never used Outline mode, have you?

If you know how Word thinks, then you can use it instead of fighting with it.

First, turn off Auto* features and Agents. Switch to Outline Mode. Bang out your content in the hierachy you want. Set up your styles. Apply your styles to the sections that need them. Insert generated blocks like TOC and index. Done. Go buy a book on Word, it's a better app than you think.
