Actually, I work for a company that provides information systems to hospitals, and provides two versions. One is proprietary, OS and all (though being ported to run on Windows Servers), and the other is proprietary software running on Windows NT/2K (and soon XP) servers.
This is actually a relevant issue for my company, and I am curious how they will handle it.
