That is not that hard.. There is an 416 processor SGI Altix at Sara in amsterdam, that is currently 41 in the top 500.

The problem with these machines is that they run benchmarks really well, but if you try doing anything real with them you do not get anything like the proper performance... That is more a problem of the Itanium2 than anything else though.
And the Intel compiler for itaniums is buggy as hell. This is bleeding edge technology:-(.
