Here in Alberta Canada the government has actually put massive funding towards ensuring that every town, city, village, community in the province has access to high speed internet (the Alberta Supernet). The population of this provice is maybe 3.5 million people and covers an incredible area. This project is fairly close to completion and offers a massive advantage to its citizens already regardless if they are cable subscribers or not.
In Saskatchewan Canada, most communities are hooked up to a provincially owned DSL line and this province doesn't even have a million citizens (and roughly half of those don't pay any taxes either). This was primarily done to help facilitate the lack of teachers available to instruct classes in remote communities, but it't another fine example of high speed internet being offered to various communities regardless if they are 100, 1000 or 100,000+ people living there.
An intersting side note is that companies up here in Canada are also already toying with the idea of shooting Digital TV and VoIP through these lines. Many of them already have broadcast liscenses to do this.
So the proof is there that it can be done if government gets off its butt and makes it a priority.

As you can expect its not easy when you are self-funded and don't have as many resources to throw at a problem like the AAA guys do. But we feel that we can compete by the simple fact that we are heavily involved with our player community (taking feedback and suggestions to make the game more of what they want to see) as well as adding new features, content and options as we move away from our Early Adopters release.
Actually Cujo, the public Linux release for Dark Horizons Lore is coming out with our next update (version 1.12), which should be out soon.
We have been offering up a beta version of the Linux release to purchasers of our game when they request it though.
