OSTicket (GPL v2)
I use OSTicket.
It meets all your criteria, and it has a lot of features you probably do not need, which are aimed at large organizations. I use it to track multistep tasks. After a year's use it has not given me any trouble.

After spending many hours researching I'd suggest osTicket for helpdesk software. It's free and open source and really good.
Just curious why you think IT is simpler now than it was before Ethernet. Or did you just mean cabling specifically? That seems kind of counter intuitive. The demands from their users on IT departments is much, much higher than it was 20 years ago. -

I tried to install OTRS a long time ago when I was setting up my small hosting business... I was never able to get it to run within Plesk. The setup for it is ridiculous, and has almost no documentation at all... I wound up using OSTicket. It meets my needs, by no means anything flashy, but it does the job.

You might try...We've had a lot of success implementing OSTicket. It's an open-source (and apparently now abandoned) ticketed support system. You can still find the code on the message board at that site, along with some modifications.
It's at least a start. It took us a little while to implement, but now it runs like a dream. It seems to offer everything you're looking for. It's PHP/mySQL based. Hope that helps!
