All posts get seen by numerous moderators. There's enough of them every day.
Some moderators are vicious. They'll see a particularly idiotic post and then check your user page to see if you're being a moron elsewhere. Bam, mod points all spent.
The IP ban happens automatically if you're modded down enough times in one day. It's not a concious decision by any admins. Hell, they don't even check stories before posting them, let alone comb every last comment.
