Once the source is forgotten, the human mind assumes that the material is original. This happens more frequently with musicians who compose the music of a tune they've been whistling for a while, forgetting that the tune was something they heard once several years ago. It still has been known to happen with writers of essays.
Also, unintentional plagiarism can occur if the writer meant to merely quote or paraphrase another work and then failed to properly cite the source. This is usually due to ignorance on the rules surrounding the proper use of quotes and citations in the bibliography.
