Schedules get blown all the time, even by good devs. Sometimes it's because the design was broken or problem misunderstood, and nobody sees it until dev or test. 'Meets schedule' always has 'buts'. Takes a coder to know an excuse from a 'holy shit, we must refactor' moment. The guy that spots the bad design first is worth gold BTW. That hypothetical 'failure to meet schedule' is a huge positive, saved everybody's ass.
The PHB can't tell the difference, so he likes scrum. Puts all the things he doesn't understand into nice tidy boxes, where he can pretend. Of course he doesn't trust geeks. Thinks they're overpaid and featherbedding, senior ones too. (He's not always wrong.) Your job is to 'manage up' the PHB. It's inescapable, there are mythical places with no PHBs, but I've never seen one. You've got to keep him out of the standups, or you are doomed.
Your people may be wrong, but they shouldn't be lying, trying to bleed you for a check (until you realize how useless they are) or transparently trying to game the system. That's 'can't trust them to take care of things' IMHO. If they're good, they'll fill any technical gaps, quick enough, if you just leave a little slack in their schedule.

IMHO the key part of the manifesto was 'hire competent enthusiastic individuals'...which can be used to validate claims of agility. Those people DON"T work for industry average. If an organization is paying industry average, it _cannot_ be agile. Most likely it's 'agility' amounts to management's ability to maintain a bidirectional circle jerk.
