i would second that ("get the management on board")
one solution, even if a bit costly on the short term, would be to hire a group of people, just like for an audit, that goes from one worker to the other and does the job of documenting all the processes that are involved. since they are hired by the organization, i.e. by the management, it is also easy to give them beforehand some guidelines you'd like to be followed. another good thing is that since they are external, they won't assume anything in the processes.
except for your #6 (which is for sure something they need more than a computer, and thus a bit odd to find on the 6st position here), a computer with internet access can almost replace the 5 first items of your list.
