Track balls are rough on the thumb. I now have osteo-arthritus as a result of using a track ball for several years.
Aside from having switched to split keyboards about 10 years ago due to CTS issues, I've now switched to game grade mouses. This is for two reasons. First, the mouses are 'lighter'; i.e. less drag. Secondarily due to the increased sensitivity I can move from one side of a dual mon display (2360x1024 aggregate resolution) to the opposite side in 1.5" of side motion.
There is very little stress on the thumb or the wrist even when actually gaming.
