If you want to build a BartPE disk, check out The Ultimate Boot CD for Windows. It's a massive collection of plugins and drivers for BartPE. Adaware, Hijack this, McAffee, defraggers, etc. Here's a list of apps it comes with.
Hands down, bar none, the best place to start your BartPE plugin collection. -

Non infected scanner?
Wouldn't it still be quite possible to scan the system from a non infected source, such as the UBCD4Win? Its a bootable cd, like knoppix and others, but with a light version of windows XP and a ton of cleaning tools. I use it regularly for cleaning spyware and viruses off thoroughly infected systems.
It's be able to cope with systems having hundreds of virii and such. If you trust it to remove simpler malware, then ingrained rootkits should be a similar problem, for an 'external' system. Not to mention it has all the critical XP system files handy for replacements. A bit easier than the 'nuke it all' aproach, which is beginning to sound like 'reboot and see if the problem goes away'.

Boot CD Trinity
There are three main bootable CDs (amonst a few others) that are in my software toolkit at all times: Knoppix, The Ultimate Boot CD, and The Ultimate Boot CD for Windows XP. All three are invaluable diag/repair tools.

PEbuilder is what ive been using.
I've been using Pebuilder for my recovery needs with great success.
It's easy to customize with plugins that you can create, download, and add. The UBCD for Windows is a must have for pebuilder and makes it a real powerful tool. from browsing to e-mail, web browsing, disk recovery and lots more. I basicially used one of these CD's as my PC's OS while I was waiting for Dell to send me a new hard drive when the one in my machine at work crashed. -
Try cdshell
Try http://www.cdshell.org/. It's a scriptable menu that you can use to boot multiple floppy images off of cds, not sure if it works for usb keys but it's worth a try. With some tinkering you can boot linux or windows live cds but if it's too big to fit in a floppy image, you can only do one of each per disk (or usb key in your case). I have a cd built with it combining http://ubcd.sourceforge.net/ and http://www.ubcd4win.com/ and a couple other tools I've found usefull
