    While we're at it, why are we still teaching kids that there are only three states of matter?

    Even when I was in middle/high school (20 years ago) we learned 4 states of matter: solid, liquid, gas and plasma.

    In fact, his account proves the opposite of his point. He mentions many times about how papers were filled out by asking questions of the patient. This informations never seemed to make into the right hands according to his story. A properly used fully electronic system would get rid of most all paper. If the VA and DoD can do it... And yes, I'm healthcare IT guy.

    The alternatives typically require a modicum of personal responsibility to ensure data is up to date. 99% of people would simply not keep the latest information in their USB Wristband PHR (or whaterver).
