Re:I'm waiting for transaction-specific codes
Some providers offer "virtual" credit card numbers (scroll down halfway) for online transactions - pretty useful when dealing with merchants you don't trust 100%.

Re:Recomendations?
I would recommend the Citi Forward Card:

I have had it for a while now and things have been good. It has virtual account numbers like you wanted that you can set either a time limit, spending limit, or both on. It has basicially everything thing else in your list as well. You can even dispute charges online without having to call anyone (just finished this and the charge was reversed within 2 days without me having to talk to anyone on the phone). It also does have pretty nice rewards anyway, fairly reasonable interest rates, and an interest rate that will drop by .75% after 3 months on-time payments. You can also set it up to auto-pay or "pay on demand" via ACH from your bank (enter your routing and account number). Anyway, I generally think of Citi as a pretty big corporate evil, but this card, so far, has been pretty good. -
Re:redirect is better
Sort of off topic, but something that might interest you if you haven't seen it before is a feature Citi offers with their credit cards called virtual account numbers. Basically, it allows you to generate different numbers that point back to your real account and are only good for one use. You can also limit the amount of time they're active as well as put a cap on how much money can be drawn from it. Pretty cool.

Re:Reloadable cards. This makes sense to me and I believe there are some services attempt to do stuff like this.
OTOG (Off the Top of Google): -
Re:YES! YES! Transparency IS solved!
http://www.verizonwireless.com/ - Menus popped under the flash box. Works now.
https://www.citicards.com/ - Flash covered all of the site obstructing login box. Works now.
I guess I no longer need a flashblock plugin operating in a blacklist form. -
Re:Rosy story, but I doubt it will play that way..Here's what the validator says:
This page is not Valid XHTML 1.0 Strict!
Their poor online support is one reason I stopped using Citi. Here's the direct link to their help page to complain about their website: https://www.citicards.com/cards/wv/topLevel.do?screenID=502
Result: Failed validation, 131 Errors

Re:What me worry
You forgot Ameritrade. (Backup tapes lost in transit.)
It's not just small banks doing what the OP mentions. Check out:

The login page isn't SSL, which means a MITM can change the form target to go to his own server when the login page loads. If an institution the size of Citibank has issues, should we be suprised that the little guys have them too? -
Citibank's web site works with Mozilla
If you don't like big corporations, open an account at your local credit union and get one of their credit cards.

Obligatory Opera plug
I wanted to spend some quality time with Open Source by running Mozilla, but my need for a stable browser overrode my desire to contribute to the Greater Good.
So I run Opera -- the free version, with the annoying banner ad -- about 75% of the time. About 10% of my surfing is on He That Shall Not Be Named for sites that don't support Opera properly (like Citibank), and the other 15% is spent with Lynx. The Lynx time rises to near 100% at work, where it's a Good Thing to be able to surf without using a graphical browser. -
CitiCards.comThis isn't technically banking, but Citi didn't support Mozilla for online credit card management until recently. About a year ago I complained to customer support and got a standard response. A few months later CitiCards.com started working on Mozilla. (Related or not? Who knows. But it couldn't have hurt.) I emailed support again and thanked them.
By the way, DiscoverCard.com worked the whole time.
Oh, and my bank ehbt.com works fine. It's a third-party thing from fundsexpress.com FYI.

Re:No Question. Report it
Secondly, I still can't understand why CC companies don't have a one-time CC# system in place. Something like S/Key would work great. You enter your credit-card number (e.g. 1234-1234-1234) and an ammount (e.g. $450.00) into a program and get a one-time-use credit-card number.
My Citibank Mastercard provides this service via an online service. I also hear that many credit cards, like the GetSmart Visa offer it via physical card readers that you connect to your computer.
