Stand Still - Powergrid
http://www.powergridfitness.com/ by far the best out there. I have used it extensively on both PS2 and PC for FPS, puzzle and racing games. This is a fantastic controller, not just an exercise machine. No I am not a paid representative.

For starters, you could buy a DDR pad and hook it up to the computer then map the front/back/left/right arrows appropriately and perhaps use the corner arrows for particular talents associated with foot movement (like Charge for Warriors). Maybe use a second DDR pad mounted in front of you that you can hit with your hands to trigger other talents associated with hand movements. The only really tricky thing is figuring out how to implement mouse-look -- a handheld mouse might work or perhaps some gigantic adjustable resistance joystick.

Kilowatt Pro by Powergrid
How could they forget the Kilowatt Pro by Powergrid?! You plug it in to a game console (PS2,XBox,Game Cube) or PC (USB) and work out. There's a stick that you push, and use it like the left controller. It's not much fun on anything but driving games, where you push harder to go faster. I imagine it works with Tetris though.

It's not cheap, but they didn't mention the Kilowatt, which is basically an exercise joystick. (you have to throw your weight into it, and it's supposed to have some adjustable resistance)
It looks like the company has a less expensive controller coming out, too.

I saw a device at CES this year called the Powergrid. It can plug into multiple game systems like PS2, GameCube, Xbox, and uses isometric exercises to get you working out at the same time. It's immovable, and you have to press forward (HARD) to get moving forward in your game. Same with side to side movement.
