I use a combination of things for "defense-in-depth"/"layered-security" purposes & with GOOD solid reasons + for SPEED GAINS as well!

Now - it's a LONG read of a reply to you here... however, it's really 'detailed' to-the-max, & specifically in regards to layered security/defense in depth + WHY (many issues listed below in DNS, browser addons, & more in malicious script online, even in adbanners):
I use a layering of these tools to achieve my goals of best possible online SPEED, & yes, also SECURITY too:

6.) Even browser addons (not as many as I used to though, for good reasons noted below (because they've been 'compromised' as far as I am concerned)) - in NoScript specifically, on Mozilla/FireFox products, & AdBlock in Opera.---
For security - Custom hosts files are GREAT vs. "fastflux" botnet C&C Servers (or other types), since the malware-maker/botnet master behind them has a "long term investment in time" in using host-domain names (vs. IP addresses), shifting WHERE & WHEN said host-domain name actually "lives" out there in his botnet (which is easily changed in a "fastflux" botnet, say, that's used to serve up a FAKE website page to infest you & yes, that DOES go on) - hence, where the value of hosts lies, for security. For Speed - hosts also can block out adbanners that suck up my bandwidth (and have been shown to infect folks online too, listed partially below) that I paid for out of pocket & can also "hardcode in" my favorite sites to get EVEN MORE speed back
IP addressed based bogus sites/servers are easily dealt with in firewall rules tables (but, this is BY FAR the lesser route used, & often ONLY as a "fallback" - but, it's also a lot more easily 'cut off' @ higher than user levels too, since it represents a "stationary target")...
External "filtering" DNS servers (many good ones listed below) are the "final stage", external to your system (I don't believe in running a local DNS due to several factors also listed below - in problems DNS has (numerous KNOWN ones), complexity, electricity usage, & more...).

No, just a hostname/domainname (no ip needed to block). To speed up a site, just "ping" it by host/domain name, ala:
ping slashdot.org
Which WILL determine this site's IP address (as an example)...
(That will allow you to speed up access to your fav. websites (which AdBlock CANNOT do for you, whereas hosts can, along with blocking adbanners (or other content)).
To block a site? Just put this type of entry into a custom hosts file:
0.0.0.0 sitetoblock.com

Same deal via the application in my 'p.s.' below I created too, BUT, you can EASILY EDIT a custom hosts file using notepad.exe or other text editors (located by default in Windows in %WinDir%\system32\drivers\etc) since it's only a text file filter for the IP stack itself...
ONE THAT MAKES ADBLOCK REDUNDANT (as is, it's less useful anyhow by far, & moreso since they 'crippled it' by default vs. adbanners blockings).

Hosts have a BETTER one - don't WANT an ad (or anything else that may be blocked in hosts) blocked?
Just open hosts (in Windows, beneath %WinDir%\System32\drivers\etc, & you MUST do this as administrator class user - save the file elsewhere if need be, & then overwrite via drag-N-drop copy overwriting the original hosts) with a text editor to do so!
One like notepad.exe & boom: Edit out that line item entry, save hosts again, & DONE!

Not hostsexpert, but rather this program -> http://www.start64.com/index.php?option=com_content&id=5851:apk-hosts-file-engine-64bit-version&Itemid=74 - to me, it really doesn't matter if you use mine or the one you note, but that you know how to 'work around' difficulties with the end-result (hosts files themselves) & more importantly (perhaps most of all) WHY they fail @ times (internal formatting on diff. OS, or other reasons) - which is RARE, but it does happen (usually, it's the notepad.exe "save as" *.txt default that does folks in, not tabs though as was the case here apparently).
I've tried HostsXpert (one MUST 'check out the competition' & all that, lol), & yes - It's alright & does the job well enough (as does hostsman)!
However, there's things mine does (such as favorite sites to speed up via SITES.TXT you edit to suit yourself & YOUR favorite sites) that others do not afaik per my last testings of them.

I've been doing the following with Ramdisks/Ramdrives below, & since 1992 or so, 1st by using separate HardDisks (slower seek/access by FAR)!
Then, later applying Software-Based Ramdrives to database work with EEC Systems/SuperSpeed.com on paid contract (which did me VERY WELL @ both Windows IT Pro magazine in reviews, & also MS TechEd 2000-2002 in its hardest category: SQLServer Performance Enhancement & SuperSpeed.com too - since I improved their wares efficacy by up to 40% via programmatic control & tuning programs for them) - which, only the past few years now it seems, OTHERS are finally "latching onto" for performance purposes in database work in industrial environs!

Distributed across MULTIPLE servers, especially with smaller data, will work well for performance!
This even works with simple text files, albeit even on a single machine (& is very effective with thread usage in combination with it on a multi-core machine)...
So - how do I KNOW this?
Couple ways, actually (in databasing first of all, but more recently, even with this):
That version of my application for host file mgt. has an "update" (version 6.0++) that I have been testing prior to re-release that is FASTER & uses something MUCH like what's being done here in fact...
The updated model takes the single, relatively LARGE custom hosts file data intake (between new data & merging the OLD data, deduplicating & normalizing it) as a single file, & essentially "shards" it too!
See - 1st, I pushed as far as I could go with it (does the job in under 7 minutes on an Intel I7 Core 3970/top-of-the-line afaik) & that was even using "tricks" like upping the CPU priority as well of the parent process thread doing that work...
Now - the new one's architecture varies on that & does the job, better/faster, so far:
I busted the data up as I process it & sent it to process simultaneously on multiple threads, 1 to a 'shard'...
(Threads for as many as the datasets I broke the single file into)
I cut the time down to THAT, albeit on a FAR lesser machine in testing here @ home!
(Intel I7 Core 920 - basically "middle of the road" in performance CPU's from INTEL nowadays, if that (it changes so far, more cache etc./et al on the better more costly chips - just like the one above, which I had a gamer I know who ALWAYS has the "latest/greatest" equipment test for me... same data I used, same all the way around, EXCEPT for my use of "sharding" (sort of))).
Thus - It shows you an algorithm/technique IS superior to "brute horsepower" which you should only use as a last resort...

P.S.=> Now, above ALL else here - I've been doing database work in information systems since 1994 professionally, & anybody that's been doing it for awhile WILL tell you this - watch it with "big" JOINS: They're an example of where things get "too hairy" & SLOW...
So, by doing this 'sharding' technique, they're doing essentially what I noted above in my app that works on a single text file, & working across multiple smaller parts that make up a whole ordinarily & seeing speed/performance boosts!
That'd only get BETTER in a distributed computing scenario (just like how commonplace multiple hundreds of PC's can outdo supercomputers in a SINGLE rig)...
Breaking it into multiple parts for threaded concurrent processing IF NOT across multiple machines, will work...

I do - It's what I do nowadays (see inside)
For YEARS now (first with this unit) -> A Gigabyte IRAM 4gb unit!
It's EXACTLY where I place my pagefile.sys (in Windows), & I noted it here (also noting that it's "iffy" do with ramdrives in the link below, UNLESS you have a "ton" of RAM & especially RAM that does unused).

That's ALL of what I do on a "True SSD" as I call it (well, almost, missed 1 part noted below...).
Thus "mirroring your idea" (my idea too, lol, from a LONG time ago when I used to do it 1st on ramdisks in software, & then later on another "True SSD" from 2000 onwards called a CENATEK "rocketdrive" but it was based on slower PC-133 SDRAM & used a slower bus (PCI 2.2.)).
The Gigabyte IRAM's based on DDR-2 RAM, & also a PCI Express buslane (faster all the way around) BUT, is limited by emulating a SATA disk though (that's where a ramdisk in software 'rocks it', far, Far, FAR faster in the respect - throughput!)).

However, modern Windows uses UAC to protect it, & I also apply read-only rights to it to supplement that, & my hosts file import/deduplicate/favorites hardcoder mgt. system I wrote -> http://www.start64.com/index.php?option=com_content&id=5851:apk-hosts-file-engine-64bit-version&Itemid=74 does the rest!
Supplementing UAC, by doing that read-only write protection attribute applied every 1/2 second & NOTHING will 'blow past that' (it's essentially locked that way vs. malicious interlopers!)
Anyhow/anyways: I place mine & all else on that list in the 1st link above onto that "True SSD" as I call it!
Just for faster loading since the seek/access is in the ns range, rather than HardDisk ms ranges!
(That's many orders of magnitude faster, for loading faster into RAM + I let the local kernelmode diskcaching subsystem cache it too, rather than the faulty-with-larger hosts files local DNS clientside cache service in Windows (known issue in it for ages, & MS doesn't fix it - so I said "hell with it" & opted to do it THAT way + I constantly am online, & that KEEPS it there, so the cache doesn't flush it out, by being online nigh constantly)).

P.S.=> And, "There Ya Go" (& 'great minds think alike') - Oh, & I "did some checking" just for my OWN reference, & it appears that 8gb ramsticks are the max size now, & considering you can put usually up to 16gb into current std. mobos, & up to 32gb into server boards? The ramdrive pagefile.sys (or *NIX file analog) IS a possible, IF you don't 'suck that up' for other things, keeping in mind using it JUDICIOUSLY for things you don't WANT flushed by say, a cache (they are volatile's why)...

B.) By using "hardcoded favorites entries" which is FASTER than calling out to remote DNS servers since it is locally done by FAR (& the 1st thing your OS loads for online access IS the tcpip.sys file/IP stack & hosts are a simple textfile filter for it, cached into RAM either by the local DNS clientside cache service in Windows, or as I do it, since that service IS 'buggy' with larger hosts files, the local kernelmode diskcaching subsystem - as well as any webbound client programs (browsers, email programs external to browsers like Outlook, Eudora & others adblock does NOT protect there vs. spam/phishing/maliciously scripted HTML mail etc. either) since the 1st thing queried is the hosts file itself, before ANYTHING else, & browser addons + browsers CANNOT determine the IP address resolution by themselves, first).
That's just a sampling - there are also 10++ things custom hosts files do for better speed, security, reliability, & even added anonymity to an extent (vs. DNS request logs + vs. DNSBL's you may NOT agree with as well), that adblock just PLAIN CANNOT DO, that is to YOUR ADVANTAGE as a user of the web!

As to advertiser b.s. & also "solutions" vs. it (advertisers owned (ghostery), or crippled by default (adblock), or full of security issues (DNS)):
Per my subject-line above:
I created a solution vs. trackers & FAR more that is malicious to end-users/consumers of website data - vs. botnet C&C servers, trackers, spammers, phishers, maliciously coded scripts on sites & yes, in adbanners (tons of evidences of that over time are listed below for your reference & that of others)...
A solution that is far MORE efficient than competing 'solutions' that are either advertiser OWNED, or crippled by default, in browser addons, no less & even over DNS servers, which custom hosts, the end result of the program below, can actually supplement & reinforce on a NUMBER of levels no less!
(Most DNS servers worldwide, especially worst @ the ISP level, remain unpatched for 1/2 a decade vs. the Kaminsky redirection dns-poisoning flaw, see link below for proof, when setup in recursive mode especially & MOST are, & have to be).
It's more efficient than browser addons OR even DNS servers in usermode, since it operates in Ring 0/rpl 0/kernelmode as a native TIGHTLY INTEGRATED part of the IP stack itself! The IP stack has over 44++ yrs. of refinement & optimization poured into it also... for efficiencies' sake as well as for security too!
(Which is far, Far, FAR faster & more efficient via tcpip.sys itself in kernelmode & loads as your OS does + fully on 1st request by web-bound client programs, which makes things like Adblock or Ghostery REDUNDANT & WASTEFUL in fact because of that very fact!)
It's better overall, vs. browser addons layering in over already SLOWER webbrowsers running in usermode/ring 3/rpl 3, & addons end up even slowing them down even more!
(TEST - Stack up a few addons in FireFox, see what I mean on that account - it is a KNOWN issue with browser addons in messagepassing alone (even if "in process"))

A lot of data on that note is below from reputable sources & a program I designed back in late 2003 that I kept to myself until mid last year/2012! Use it in other articles you may write later on this very subject (and the program is 100% free no strings attached should you elect to try it as well to use it also)...
You seem to be as "into it" on this note, as I am (almost, you'll see what I mean in the program below). I went & DID SOMETHING about it, for others that they can literally use & yes, it works better than anything else does for that purpose!

Per my subject-line above, custom hosts files = superior to browser addons!
(Since hosts are a TIGHTLY INTEGRATED native part of the IP stack itself, running in Ring 0/rpl 0/kernelmode, vs. layering ontop of far, Far, FAR SLOWER Ring 3/rpl 3/usermode browsers, slowing them down even more (known issue, stack a few FF addons up, & see for yourself))!
In fact, Proxomitron is 1 of the FEW tools like it that actually works WELL with custom hosts files that I know of!
HOWEVER: Proxomitron http://en.wikipedia.org/wiki/Proxomitron bills itself as "the universal filter"... it's NOT AS UNIVERSAL AS CUSTOM HOSTS FILES ARE!
(Which again, are part of ANY OS' IP stack that's based on the BSD original & most all today, if not all, ARE based on it - even on smartphones, & does Proxomitron run on those? Sadly, you're wrong (per my subject-line above) since AdBlock doesn't block all ads anymore, by default (& can't do 10++ things custom hosts files can, listed below) - same goes for Ghostery & even DNS servers!
