There is no reason that cable shouldn't be REQUIRED to service these areas if they want a monopoly.
Who should be required. Where I am in WI, there are at least 3 different cable providers in the state. My village has cable and broadband. 1 mile away, I leave the village and enter a township. They have no cable at all (so, obviously, no broadband). Since cable companies negotiate their monopoly with each municipality (at least in WI, it isn't state wide), who is responsible? Time Warner? Comcast?
I don't disagree with the statement, but I don't see how to enforce it...

If you don't like it, don't pay for it. It is pretty simple. If enough people refuse to pay for it, it will go back to being free. Or are you concerned that enough people will pay for it and you won't have a choice?
One interesting comparison someone pointed out to me is this: people think of Microsoft as a monopoly. But can you imagine them charging you for a "loading Windows sound" the way telecoms charge you for ringtones?

That isn't the best comparison. You are talking about MS charging for something that used to be free, where in cel phones it didn't exist and it was being changed from the beginning. This of the first PCs. They had no sound except a speaker (which was free and provided free sounds). You have to purchase a sound board to get the sound (which is not generally built into the motherboard).
And of course it's closed-source, which means it has few advantages over several other alternatives.

Legally, I suppose it would be within your right to create a fork under GPL2 but ethically and morally it would be stealing since the original copyright holder (Michael R Sweet) was the main contributor and any other patch contributors assigned rights to him before they were included in the repository. You would basically be carrying out a coupe and violating the spirit of the license if not the letter of it by taking something none of you owned and creating a fork of it.
