I believe they are stimulating the ganglion cells in the retina. Those are the cells that take signals from photoreceptors and send them to the optic nerve. The primary target for such implants are people with macular degeneration: the photoreceptors slowly die, but the rest of the retina is healthy. This is the leading cause of blindness in the world.
Also, you don't necessarily need thousands of electrodes. Even with a single electrode a person can potentially learn to scan the scene to identify objects.
