Part of the problem is the engineers designing them. They don't understand the sandbox they're playing in. It isn't in their culture and they don't know that they should secure them much less how to. I'm starting to see organizations hire product security engineers now to try and institute this stuff into the products but they seem way behind the curve IMHO.
You can't remove the blame entirely from the players, especially the pros. Most of them want to do what they do. They're attracted to the big money, and there's practically no way they'd make that kind of dough in any other endeavor. Plus, there's the machismo factor.

The biggest problem I see with 80% of my peers in this religion is they're catholic exactly 1 of the 168 hours in a week. Most of them not even that. The other 167 hours are spent judging others and just basically leaving the world in worse shape than how they found it.

It isn't uncommon (actually quite common) for stroke patients to be done over a video conference . Why should psyche be any different? I would think the doctor would and should be looking for subtle cues and body language just like a Stroke Neurologist is looking for signs and symptoms in their patient to decide a treatment plan. A clot buster drug given when it shouldn't be can be fatal.
What if the person likes to cut themselves? He might want to see those signs.
Yep. I had an older but still running Toyota pickup. I sold it to a couple of guys who were taking it to Guatemala. Make them come to the bank first so they could certify the bills were not fake as they insisted on paying in cash. DA's office said no known scam going around like that but it was a little freaky to say the least.
The last company I contracted for made me work two days after I had back surgery. I fielded calls from underlings while in the ER. Good times.
It happens just like you say. True stories from previous employers:
Father passes away. At funeral home receiving friends and family expressing condolences. Boss drives 120 miles round trip to express his condolences and while he's there, not 5 feet from Dad's casket, hands me a piece of paper with DNS changes that need done. No one wanted to learn it and he was way too willing to stick his head in the sand and not deal with it.
Getting married the next year. Reorg happens and I get a new boss. Have honeymoon and wedding vacation time scheduled with previous boss. New boss tells me -- I didn't get vacation from him. Resubmit or I can't go. Told him to go pound salt. He did and I went on vacation and got married. He was going through an ugly divorce so maybe he was trying to do me a favor.:-D
These are just two that are directly relevant to this thread. I have many stories of patently dysfunctional things done to me by bosses that make me understand why lawsuits happen.
