The way you discribe it, the problem seems to be that the little guy has to be a little more innovative then before, because making minor changes to existing ideas just doesn't cut it.
So when Rajiv Six-Pack comes up with a really brillant idea, nothing - nothing is stopping the "Microsofts" of the world taking his idea and making millions from it.
Or do I really misunderstand the benefits of No Software Patents.

That may well be the truth, therefore, lets say that output of US Scientist is 3.6 this puts them in the top TEN, of course letss start looking at the relevence of the output of the other nations.
The second place UK, would only require a rate of 71% trivial collections to match 3.6% in the US. For Japan this would need only be 52% t.c. to equal that of the US, as for there rest, Canada who on RAW output would be ahead of the US, would only need to generate 23% trivial collection to equal the US.

I feel reasonably confident that the US would be in the top 2 for output of scientific publications based on the premise that all nations product SOME amount of trivial publications.

I reiterate that the respondants are allowed to be subjective as it is their opinion being sought.

It is up to the researchers to then publish all the opinions in the form of statistics which can be used to form an objective evaluation.

Also it really up to the respondant to be objective?
I would think not, because by the nature of surveys is to canvas subjective opinion and then present all opinions objectively.

This really depends on the level of understanding the developer has for the client needs in the first place. It also depends a lot on the clients ability to properly describe what they need.
Since these are often dynamic, then the mix of planning and documentation vs protoyping has to be determine for each project.
The biggest issue with prototyping is when the prototype is seen as the finished product, no matter how well it is explained that it is just a visualisation tool.
