A prominently displayed tattoo on the wrist or chest (or both) would be visible and actionable well before getting to a hospital room even if the patient was non-responsive.
Also, if someone is in the ER there is a good chance they didn't teleport there either. The EMT's who scooped the person up would have had the opportunity to see and report on the DNR orders. Triage would surely discover this information at the hospital, if it were commonly in use that is.

Medical personnel could then input the reference number and download the properly filed and legally binding documents. If a hacker just happens to open that stream of communication from foreign soil and you just happen to be eavesdropping on them you might just get some actionable intel on your domestic lawmaker. Telling said lawmaker about the attempted hacks would seriously undermine your ability to legally spy on them, so there's no way the FBI would do that.
