I was already planning to pay for it, I just wanted to get him to admit it wasn't free. We went through the cycle of free, not free at least 15 times until it just was sad and I was ready to agree to my free $9.95/mo HD upgrade.

TNT does it, at least for their Comcast HD feed. The local news screws up sometimes and broadcasts their (normally widescreen) news in the 4:3 ratio, then stretch it back out to 16:9. It's obviously just a screw-up in the second case though.

I don't mean that Tina Meyer was a bad person at the time of Megan's death. I think Megan's death made her a bloodthirsty person though. Lori Drew screwed up, and she'll have to deal with that. I do think that enough of my peers who are parents now either encourage bad behavior, or demonstrate enough of it, that they're a hazard to their child. In addition, many get downright scary when their kid really pisses them off.
I think the Meyer family meant incredibly well, and they were dealt a bad hand, but I've seen plenty of teenagers freak out when their parents tried to punish them. I've also seen parents freak out from a basic "That's not fair!".

For public space WiFi networks it's a fairly simple solution that intercepts port 25 traffic, and throttles you to x per minute. It's designed for networks with many mobile users.
Considering these are affordable for many types of smaller businesses, it's hard to believe that all consumer-grade ISP's can't implement a similar technology.
(Yes, I am aware that there are many DSL providers that all SMTP traffic goes through them, but there's obviously a lot that don't.)
