In those three cases, the pilots lost all engines, and acted in contravention of the designed rules, and saved 100% of the people. When simulated under textbook process, all three had fatalities. The humans improved on the engineering while flying flying bricks.

Gimli pilots should have broken standard procedure to re-check fuel load, as it's the pilots "responsibility" to take 24 hours to check everything the ground crew does in the 15 minutes between flights. And the SA751 pilots should have manually disabled the engine management features they were unaware existed (due to poor airline training).
So the government exists to provide jobs to vets? The government exists to do nothing but be welfare. Seems they should try to be more productive.
And when the government does as you say, and hires an unqualified applicant because of demographics, they will perform more poorly than the private sector. Which is one of the prime complaints against them.

The vote-checkers that most polling places have you scan before dropping it in the box should catch some of these errors, but not all, and are also vulnerable to tampering.
Unless you are using pen-on-paper ballot marking (tick the box), or an electronic system with multiple printed receipts, it's *impossible* in the US to verify how your vote will be cast.
At this point, we'd be much better off going back to open voting. It worked great, until the civil war, when armed poll workers would kill you if you voted wrong (or looked wrong), though such tactics would work now, and that they aren't indicates we are mostly stable enough to go back to open voting, to fix the massive problems we have now.

A picture of a form with no context is proof of nothing. Most states require proof of citizenship to get a state ID. Some places will allow a person to "vote" without an ID, but require proof of citizenship to get on the voter rolls. So your photo of nothing shows nothing and proves nothing.
