Have you ever heard of DLL Hell? The situation is the same on the windows side, just that most apps include their own version of the dynamic libraries in their app, which unfortunately blow away whatever you have installed and lead to an unstable system. It's hard to manage different versions of these libs.

There's no reason that a HD means that patching will happen. Especially if the OS is in a ROM. Patching occurs because of sloppy programming and can happen anywhere. Remember the PS2 was recalled in japan, and that many console games have bugs (not nearly as bad as the PC market however, probably due to homogeneity).

What we need are fully scriptable apps so that the GUI presented can be completely modified depending on the user preferences. Not just 'skinning', but allowing new functionallity by binding keys & menus to script macros. That way a beginners interface could be the default, and you can tweak the interface (or download one) for more advanced users. Emacs and Autocad are two apps that come to mind, though I wouldn't say the execution is perfect in either.
With tools like python and guile that are easily embeddable in other programs, i can only hope that this is on the horizon.
