I think right now the biggest thing I see keeping away commercial developers is the lack of a single binary installation/uninstallation API integrated into the desktop environment.
I couldn't agree more. My biggest problem is related to this, it's dependancy handling. In order to install App X you need XLib, that needs XLib-Devel, that needs XXYLib, that needs XXYLib-Devel, and so on. When you go more than 2 or 3 levels deep in depends it's nigh impossible to install the App you want.
It has a long, long way to go, most of it internal infrastructure issues (the fact we're still using X11 is embarrassing).
Why? X11 isn't perfect, but there are perfectly valid reasons to use it. It's powerful, it's stable and it's well documented.

It's kind of an ongoing thing. That's why people get sent to nursing homes. Some elderly people don't have all of their mental faculties anymore. Some of the elderly need to be cared for, so they don't hurt themselves and so they are not hurt by others.
It was about 5 or 6 years ago, but I remember a case in my area (Western PA) where some guy picked up an elderly woman who had Alzheimer's disease and convinced her that he was her deceased husband then spent the next 4 or 5 hours screwing her brains out. He dropped her off none the worse for wear, but things could have gone a LOT worse.
Something like this will make a nursing home more attractive to the relatives of the people who need this kind of monitoring.

Movie theaters have contracts that they have to abide by, one of the provisions is not letting people see the movie without a ticket.
