Uncapping will become more and more difficult as time passes. We're a DOCSIS 2.0 ISP for a national cable company. The firmware we use on our cable modems only allows tftp access (the method cable modems get their config file) through the RF port on the modem. Older firmware / modems would allow tftp through either the ethernet port ot the RF port.
Once an hour, modem data is collected, and checked against our DB of configs. If there is a difference in the MD5 checksums, the modem gets bumped, and the customer notified.
We also do shutdown modems that are passing viral/worm traffic. Customers get one warning and 24 hours to clean it up.
We've also started blocking out bound SMTP traffic to other then our servers (There are ways to request access if you host your own email elsewhere)
Oh, and the writer of Beagle.k is on my shitlist. The number of support calls that the call center had to take because of this virus is amazing.

I work for an ISP. We have a simple, inexpensive, yet effective solution. No outbound traffic destine for port 25, unless it routes through our mailserver. Users need to authenticate against our mailserver as well.
Corps that host their own mailserver, must use one of ours as a mail gateway.
As for "The ONLY way you are ever going to get the ISP's to spend money to implement this filtering/quarantine is if you sue them for allowing their infected customers to cause harm to your business."
This is probably the best way to increase your monthly costs for connectivity.
If you want to sue someone, sue the Luser that executed the virus on his machine.
How about the realistic approach, protect your own networks. Make sure none of your users spread any mess. Block incoming attachments on your mail gateway. Understand, not al "ISPs" provide what your AOL or Earthlink account does. Some ISPs supply DS-1 and higher connections and IP Block leases.

Me: Techsupport. How can I help you? User: I received your email about needing to patch my windows. Me: Sir XXX Company will never send you a file by email. It is most likely a virus, please delete it. User: Oh, OK, so I shouldn't double click on it (Audible CLICK CLICK). Me: Sir... User: Oh...It's ok... It doesn't work anyway. Thank you... Click.
Many times we've used a piece of open source software at this company, and paid the creator to modify it. Any changes he makes for us, are allowed to back into the open source project.
Yeah cause hi-rag(no not exact obviously) content paper isn't available at staples, and the 16 year old at the window at McDonalds can tell the difference, or even cares. How many times does the street vender look at your $10 bill when you buy a dirt water dog or a pretzel... He shoves the money in his apron, and reaches (usually with the same hand) for your food.
