Bah, I'd argue that it was unintentional, and it's not "segregation". Yes, the "war on drugs" has been a failure, but I believe it was well intentioned. We all know now that it's caused a ridiculous incarceration rate. But think back a few decades if you can. The vast majority of the population believed that pot was BAD, or that it was a "gateway" drug. Do you believe that we were fed this information in order to incarcerate more of the black population? Certainly, there were plenty of Caucasians doing drugs as well.
It does. I have DNA test results (Medimap...hopefully the price will come down, it was ~$600) showing which meds work best, and which don't, in a sort of stoplight chart, along with verbal descriptions. The results were enlightening, and explained why a couple meds weren't doing much for me.
I was in a trauma unit last spring, and given morphine, even though I wasn't in much pain initially due to severe numbness...it was a prophylactic (no, not that kind). My nose was nearly severed from my face. I was sent home the next day (thanks to a great surgeon) with a script for Percocet, and told not to wait for pain to take it. For me, it didn't end up mattering because the nerves are still damaged, leaving me feeling as though the area was injected with Novocain...all the time.

Contracting officers change frequently, so they don't often understand the history or previous methodology on long term projects. This causes churn and additional bid work. Frequently military contracting officers are just out of college with little to no experience, and controlling huge budgets. Requirements documents are always lacking. And the number one reason...politics. On any large contract, there is political pressure to go with certain vendors because they happen to be in the districts of representatives, or states of senators who have clout over that agency.

Dealing with yearly bidding on a huge government contract would be very inefficient...as is the government bidding process. Transition between companies is no small deal, and typically can take a couple months on a large contract. Do that every year, and you've lost any cost savings you might gain on a longer contract.

Why not treat them as public utilities...I'm thinking of them being like many electric power companies. They don't have to be non-profit, but they are heavily regulated to control their local monopolies.
