This for is an affidavit. 4: Any contested charges are investigated and action is taken on a per item level.
A few points to consider: In regards to action 2, any legit charges that have not finalized on card x (the lost/stolen one) WILL hit card y (the new one).
On point 3, if you lie on the affidavit you WILL be charged with credit card fraud. Your credit rating will go right down the shitter and you will probably go to court and jail.
