1) The FSIS inspects meat, not the FDA. This is a binary pass/fall system.
2) USDA grading of meat is a VOLUNTARY process, there's no reason to get "their own grade of meat".
3) McDonald's ground beef is made from a mixture of fatty domestic beef and lean, mostly imported beef. I think this is done mainly for the sake of consistency but the fact that it's cheaper this way doesn't hurt.
