The name of the product was actually "Ultimate Quake"... it was something of a limited edition, I believe, and had a Quake 4 poster included (which is now on my wall.) Hopefully the clarification helps you in your hunt. Amazon has a listing for it and sometimes carries it used, but they're out right now.
Grab it from the Id Software FTP or from mirrors at the Quakesrc forums. Offtopic, I know, but my submission was rejected. If they have time to reject the story, they ought to have time to approve someone who submitted it.

There's actually a boss in Kirby Superstar, which is not a roleplaying (or stat-building, if you prefer) game, that enforces the rules of turnbased combat.
The following all takes place in the context of a standard 2d platformer:
The boss is invulnerable while it's attacking. Whenever you or the boss executes an attack, a dialog box pops up in the background that reads like:

Randomly generated levels are the holy grail of replay value. Diablo 1 had them; Diablo 2 may also; haven't played it. I can confirm, though, that Diablo 1 still hasn't gotten old for me, because it's different every time you play it.
For a long time, I wished for a Super Mario Brothers game with randomly generated levels, but then it occurred to me that if such a beast existed, people would never have to buy another Mario game.
This oughtta do ya fine. Haven't tested it though, so it might be down. The web page says there's a good number of seeds, though. So start your engines.:D
By the way, I've found that emule downloads go faster during the second half, when you have parts of the file that most others who are downloading the file don't. It's that credit system.

The only complaints I have with these are due to their handheld format: The small screen results in low resolution compared to symphony, and there's no way you're getting playstation quality sound out of a Gameboy. The first can be remedied to some extent by running the games using a gameboy emulator on a modded xbox, and applying some anti-aliasing, but you'll have to acquire a taste for chiptunes in order to get past the music.
Circle of the Moon has the best music, Aria is the best game, and Harmony, while not bad, is mainly played when I get tired of the other two.

Oops, my tag is open. How embarassing.
With a little work, a modded XBOX comes close to being what you describe. Check out all the emulated systems. Admittedly, you have to mod it and set up the emulators, but after that, it is very hassle free, my cousin, who is an elementary school student, is working his way through Super Metroid on a modded xbox as we speak.
