Michael Malone pretty much built the company up from his garage. I remember signing up with iiNet in the early days, when it was the first in Perth to offer PPP access and then later, ADSL. It was also the first ISP in Australia to roll out its own infrastructure to allow 'real' broadband speeds (1.5 MBit/s+). Given his track record, I guess he has a pretty good handle on issues concerning the ISP business.
Talking about reform, I find the most illogical thing of user interfaces is the menubar.. how do you exit? Go to "File". Where are the options? Under "Tools".. why can't somebody offer a totally new way of making the menu. Start with "Program", where you have "Options" and "About" (maybe "Help" too), then "Document" or "File", and then "Edit", etc.. We're so used to File -> Exit that we stop thinking how illogical such a construct is... exit the file?
I believe they are trying to address this in Vista, going as far as removing the menu bar completely (I guess relying on the red X for program exit). Check the following blog post:

One task that consistently stumped users, causing them to go into exploration, was the initial discovery of where the web browser resided. The seemingly obvious answer eluded users for seconds.
I can't help but think that GNOME usability would shoot through the roof had they put a porn icon right on the desktop.
Anyone else quick read the title and expect this to be something about Doom III? I hope Carmack is well beyond spinning cubes at this stage of development..
