    AN ONLINE trader who cheated the taxman out of more than £300,000 in VAT receipts has been given a suspended prison sentence. For five years Daniel Waslin, 35, failed to register his business for VAT as he made more than £2.6 million from the sales of remote control golf trolleys and garden furniture imported from China. He evaded paying £323,000 in VAT. HM Revenue and Customs found out about his online auction sales through import checks and he was prosecuted for tax evasion.

    Until somebody can solve the big problem with all digital currencies they are destined to be technological playgrounds and not true digital currencies.

    Currently all digital currencies are extremely concentrated in very hands, much more so than even fiat currencies.

    Power might of well have been the trigger, but the scale of the outage indicates much bigger problems afoot. The root cause of this turning out to be poor IT Governance, those big picture processes that prove the resilience is designed in, tested and proven. That is a failure of IT management.

    IT treated as a cost centre, everything is outsourced on a lowest cost basis. Those suppliers are further whipped into line by crude metrics by managers that got a leg up by doing things quickly or cheaply, not properly. I see this kind of lack of concern for proper governance every day, address this lack of proper governance is by far the most difficult challenger I face as a consultant working in QA.

    The NHS failure was exactly the same thing, the attack was the trigger, the root cause of the collapse of IT was governance failure by very senior management failure to ensure resilience was built in and proven.

They have been increasing aping his methods and approaches creating troll factories that combine human trolls with large scale botting and hacking, fake news memes. This has happened with Brexit, Trump, the rise of Le Poison Pen and in the current UK general election.

    This should be all but impossible on a competently managed system, it should not be able to proliferate to this extent on computers in a controlled environment. The scale of this attack is troubling. The fact that this could happen is not a single failure, it is major failure of IT governance. Layering controls in platform and process with should prevent a single technical or human failure compromising the whole system like this. The compromise should always be localised. This is a governance and senior management responsibility and failure. If they knew this was a problem they failed and if they didn't they failed.

NHS Digital is national patient administration system, it is bold in scope and vision but with a history of expensive failure and delays caused by miss-management by practically every major IT consultancy that exists. I never worked on it myself but know many colleagues that have and non have ever had a good word to say about it.

    The real problem is poorly considered and expressed requirements and the resultant feature creep. This is failure of governance, not a short coming in developers. Those saying I double (or whatever) their programmers estimates need to look in mirror and recognise the failure is their own.

    So no, I do not have any trouble giving accurate estimates, but then I refuse to give any until the requirements are well defined and understood by the team. I my experience this is norm now for most genuinely agile teams.
