The lack of proofreading by Verity is rather amusing, as I was not aware that MS SQL was available on either Linux or Solaris, but I digress. I'm not sure which version you used, but Version 5.3 (June 2004) does not appear to use an external DB, and 5.4 (released June 2005) also does not appear to require an external DB, and does not appear to support MySQL out of the box. You're missing by far the biggest intranet vendors. Verity is the king of this market, and have been since the mid-90s. They get a lot of mileage out of their OEM sales; it sounds simpler to a company if they hear that they "already have Verity" within Documentum or Cold Fusion or whatever.
It'll be very interesting to see what they do with the Inktomi purchase. (They bought the productized search before Yahoo snarfed up the external services.) Inktomi is IMHO the best intranet search engine right now. (I believe Verity is dropping the Inktomi name and is calling the tool Ultraseek, which goes back to Inktomi's acquisition of Intelliseek.) The purchase gave Verity yet another leg up with enterprise search, it'll be interesting to see if they leverage the technology or if they see this more as a marketing move.

Searching
Most CMSs actually don't really bother about searching via their internal datastores, mostly because of the problems you've raised above.
What you tend to get are two (not mutually exclusive) approaches:
- Spidering of content using a packaged solution from vendors such as Verity, Autonomy or Thunderstone
- Internal datastore searching of taxonomy - metadata about how the content is organised. This is the harder one to crack as an effective taxonomy takes a looooong time to get right for a half-way complex site.
Sidenote - you can get around those query string URLs with most serverside scripting environments (as well as that PHP tutorial, it's worth bearing in mind that evolt's site is built in a similar way using Cold Fusion)

Sorry to inject a little bit of reality, but...
I can't see how immediately the majority of people will jump directly to the most paranoid inferences anyone could draw from the capabilities of such a product.
I mean obviously whatever evil corporation you work for may choose to abuse this type of product as another way to spy on employees, but be realistic - for the most part companies don't care what you do as long as you're productive and don't cause problems. Sure certain environments are very high on increasing productivity, to the point where washroom breaks are timed, etc, but most places are not like this.
In a less orwellian workplace, these types of products can be excellent tools to facilitate the search for knowledge. Would you know who to call in your company to get some obscure piece of info about how a particular system works? Would your co-workers? Probably not - that's why this type of global content indexing and affinity-mapping can be a great asset. I may not know who to call about the way an ACF2 subsystem on mainframe X is used by web application Y, but I can do a quick search and have someone recommended. I would kill for that capability in my job today - as an enterprise architect we need to know who the movers and shakers are in the company and keep a handle on ongoing projects, and unfortunately 80% or more of them never go in front of any sort of review board, so searching out the stealth projects is a big problem.

on Search EnginesI wouldn't concentrate too much on what search engine software to use, since it didn't really sound like you'd built out much of a requirements list of what you wanted it do. Think about how you wanted it to function and then look for the tool that matches accordingly. I would even go so far as to create a search page, and a results page. A good place to start would be searchtools.com.
As for building out a Topical or Hierarchical structure like Yahoo's or DMOZ, you need to have meta data about the document. You can pull meta data from a URL, as was suggested earlier, but I wouldn't advise that. In order for this to work, your URL's start to look like 'http://city.gov/CityCouncil/MinutesOfMeeting/2000 /10/15/index.htm', which also makes it difficult to move things around, and makes it difficult to return the string 'CityCouncil', properly formatted when you build the hierarchy on the fly. If you include the hierarchy information in the page, you can get around some of these problems. I think the standard way to do this would be using meta tags. View the source of this page for an example of this. The downside to this is that you have to structure your hierarchy in advance, which goes back to getting & building requirements early, you also have to convert old documents to include these tags, and you have to make sure that future documents will have these tags properly implemented. If you're using URL paths to qualify the documents, you may be able to take advantage of an already existing directory structure to build your heirarchy. I just don't like using document locations to figure out what a document is about--its always seemed like a brittle solution to me. On the other hand, I've used the URL string to help make a first pass at placing meta tags into documents, usually using a Perl script. I still had to go back in and check the documents though.
I've used both FreeWAIS-SF and Verity for implementing searches like this, as well as home-grown solutions. I wouldn't advise using Verity since I think that it's prohibitively expensive, unless it has some feature that you require and are willing to pay for it. And I didn't think the home-grown solutions worked as well as the off the shelf products that were customized.
