The problem is with having a large enough field with a large enough resolution that you can scan the sky fast and process the data automatically. A camera with a huge resolution is perfect for the job.
There is no punishment harsh enough that makes people feel better after losing a family member. But that is not the real problem since you can't take care of every person in the world. All we can do is make sure that on the whole less people die. Regulating each possible situation that may arise while driving is naive at best.
Now, how are insurances helping to weed out the high risk drivers? You just obligate dangerous people to pay in advance for the damage they will cause. And usually the insurance they pay doesn't cover a single repair cost. The money actually comes more from good drivers too since there are probably more of those out there. Also, insurance covers only some of the possible accidents and certainly not moral damages that would be payed in some extreme cases.

Whatever security and safety measures we put in place there will always be someone stupid enough to harm him/her or someone else. The server can just send to 1000 people and let them send to the others. The ones that receive the stream last could have a big delay (minutes, but hey, you have to compromise something). You'd also need a client that knows to prioritize the chunks at the beginning so that the movie flows without interruptions.
