Basically all of these are attacks on the end user, what you do or don't do on the server won't change a thing.

If the daylight saving time boundary moves, the dentist will not go through all of their appointments and tell them that they've been rescheduled by an hour because DST started earlier/later than they had expected. Your appointment is still at 3PM, no adjustment is needed.
Furthermore, if you use an external library for conversion (for instance, libc) then there is a serious risk that the library is updated to the new conversion factor before your users have a chance to run your script to "re-adjust" all of the appointments made under the old conversion factor, leading to some appointments being right and some being wrong, with no clear indication of which.
In fact, when communicating across time zones, using the originator's local time and timezone for future dates is preferable: if part of the world revises their DST and another part does not, then a conversion under the old rules between the two time zones might be wrong but there's no way for the receiving computer to know that.
For dates on disk or in log files, then a standard date such as UTC is perfectly fine, after all, it's not like the government is going to go back and retroactively change the start/end of DST... right?
But future events... if I store an appointment in GMT, and the government declares that the conversion factor will be different by 30 minutes, then I've got to go back through every stored appointment and revise them, before I allow anyone to add any new ones to the system. Especially on a Unix system, where timezone data is usually centrally administered and was automatically apt-got weeks ago before anyone thought about running the script that changes all the appointment times by 30 minutes.
Any sufficiently enterprisey encryption system would have a site-wide "master key" entrusted to whatever IT staff is responsible for rescuing people from forgetting their key.
