We can be kind to our friends in Canada and only charge them 100 or 200 percent because they have such a wonderful healthcare system. Rich Saudi princes or Hugh Chavez can pay say 10,000 percent because they can afford to pay. ANd since the "rich" should pay their fair share someone like George Soros or Bill Gates can also pay that same 10,000 percent tax on medical services.
