A decline is when the issuing bank will not pay the charge attempting to be authorized by a merchant. The merchant will not receive funds for a decline. Chargebacks occur when a card number is found to be stolen (or some other reason) after a successful authorization and transaction, or if a customer files a protest for a charge on their credit card.

My company often receives mail orders containing personal checks. Every year we see several who continue to allow their SSN to be printed on the check, along with the name, address and phone number. These folks are seniors (+70) and don't understand that the SSN is the key to the castle.
In a few cases, I've sent a short letter explaining why they shouldn't do that, and a few have called and thanked me for the information. Often, the reason they give for using the checks is they have a ton of printed checks and don't want to throw them out. Of course, if they did, that could be even worse than continuing to use them because you know they don't own a shredder.
Indeed the dynamic IPs are on every legit blacklist known to man, but the emails have still been sent and we, the users, have to deal with the slow connections, etc,. as a result.
Would it be too much to have Micro$oft add a function to Windows that would prevent any port 25 outbound traffic without explicitly entering a passphrase or something similar? That might go along way to stopping Mom, Dad's and Jr Hacker's computer, which is infected from all those free offers, Myspace and gaming sites from sending emails?

The USPS has improved its mail handling capabilities over the years and they do conduct tests to measure themselves, but they have a ways to go. Your mail may have been delivered, but you have no idea what has been sent to you that you never received.
While the USPS is a stand-alone business, its roots are deep in the Government. Before they were a business, they were a joke in the logistics business. I don't want my ISP to be a government controlled entity. I want some regulation at the local level to make them more accountable, much like utilities are now, but not owned or managed by some hackerama of Federal politicians.

As with credit bureaus, the SSN is not the primary method of ID, but it helps sort out people with the same name. Medical records are far more detailed than your credit history. You'd be amazed what's in them.

I'm a merchant that accepts all major credit cards, and no, declines are not charged a fee. Only successful authorizations are charged.
