Why would Walmart pay extra money to it's employees when the Government will pay them? Walmart gets to keep millions and the Government hands out food stamps. It only really works if everyone does it, hence the need for minimum wage.
When work comes in spurts and bunches you can look forward to the downtime in between. It can be a reward for getting stuff done. You can think that if I get this work done, I get a small break after or, if I work harder and faster to get it done sooner, I can have a big break. Think of roofers shingling a roof on a Friday. You don't see anyone standing around, they are on each others asses and by 2:00pm or so they are done the roof, packing up and starting their weekend early.
When you have a constant workflow that never ends there is no real incentive to work harder. You look around and see one guy doing the bare minimum and another guy doing 3 times the work load. Both get payed the same amount and the work never ends. The hard worker might think he is more likely to get a promotion but management thinks if we promote that guy, we would need to hire 3 guys to replace what he does. Lets keep him right there so we can keep our production numbers up.
The worst thing management can do though is fill an employees down time with more work. Basically you have punished a hard working person with more responsibility and work with zero pay increase. Unless you are trying to kill productivity.

If a cable company is servicing a neighborhood of 1000 houses, it cost the exact same if 10% are customers or 100% are customers. They will charge you up the ass because most people will pay for it but if you threaten to go elsewhere they will give deep discounts to keep you. It's called customer retention. It's better for them to cut a bill from $100 to $50 because $50 a month is better than the $0 a month.

When bugs are discovered, they are factors that were never originally considered possible. People with names longer than the character field, leap seconds, changing daylight savings hours, operating system changes, network growth, hardware upgrades. Regardless of the developers experience, no one will be able to account for these unknowns and how new code for new features will interact with older code. Bugs usually are not from screw ups but from changing factors beyond anyone's control.

How many more copies would it have sold? Also, how many of those 2 million customers think that Simcity 2014 or what ever the next version will be called, is going to be worth the same hassles?
I'm gona throw in a nomination for EA and the launch of Sim City. While probably not the largest screw up, I would say they had the most warning. With security breaches or even building a new Health care website, there are unknowns. You can't predict when someone is going to steal your data or how they are going to do it outside of a few tech guys that know how their systems work and whos warnings go unheard. EA had everyone screaming at them to not use DRM but they did anyway. They were warned that if they did use DRM that servers would be maxed out on launch day. They claimed that they were prepared for it but obviously they were not. They were warned and people begged for them to listen but they didn't and come launch day, everything they were warned about happened. It wasn't a minor hiccup either as it took a month for everything to be sorted out.
