Some of those small ISP's were truly horrendous, wildly overselling the bandwidth they had wholesaled (some whas hidden by crappy dial modems and bad phone lines), their employees would also wholesale spy on what their customers were doing as some kind of ego power trip.
As time progressed the true value of the internet became apparent and the incumbent telecoms attempted to drive smaller ISP's out of business, those telecoms also saw the developing opportunity to become monopolists content supplies by driving all other content suppliers out of business. The telecoms would supply the content they were 'publishing' free of data charges and then add data charges to any other content supplies often those data charges far exceeding the price of the content, so either 'publish' via an incumbent telecom or go out of business.
What buggered it up was the telecoms greed, they were not working with the RIAA and MPAA et al, as far as the telecoms were concerned they were competitors to be driven out of business. That pot of gold, the trillions at the end of the rainbow, mega bonuses lit up those piggish little eyes of psychopath telecoms executives and it fell apart.
We are now looking at stage two where the sociopathic greed of the telecom executives has been stifled and rather than competing with the RIAA and MPAA they are working together to shut down forever independent publishing. Make no mistake it will be cheaper to send a DVD via post than electronically upload it, including all associated costs. So no independent movies, music or games, even larger web sites will be threatened (by larger, I mean anything in the top 10,000, maybe even 100,000 foreign or domestic).
So electrical free market. The existing electrical supply company owns all the power lines for say 200 miles. You are free to disconnect and pay another company say 200 million dollars to run a power line that distance and perhaps a couple million dollars per year maintenance fee but hey you get cheaper electricity.
So free market never existed for essential services like roads, water, sewer, storm water, electricity and wired communications. To privatise any of them is to immediately establishes localised monopolies, who are free to charge what they want, subject only to government regulation, all based on no alternate realistic method of supply.
Privatising essential services has just proved to be a profit bonanza for often foreign corporations (less opportunity for effective protest) to screw over populations. It is a lie driven by nothing but psychopathic greed. Pretty much any person that calls for privatisation of essential services should be subject to immediate investigation, it's called behavioural profiling, you can be pretty certain that they cheated on their taxes, are involved in offshore tax avoidance schemes, regularly break environmental laws, break labour laws at every opportunity and fiddle with the books to inflate their bonuses.
Really, Really dumb. This is about reality, you pay top dollar for access and still get crappy service, why you don't know. bad modem, bad software, your ISP suck, some in between ISP sucks, the sites you access ISP sucks, the site has bad servers and can not handle the traffic.
Basically all of them can bullshit you all of the time and no matter how much you pay sell you bugger for years, until their systems are independently audited. Far simpler to set rules and make them adhere to those rules. No monitoring other than for occasional and only as required quality of service. No alteration of data in either direction. You sell the bandwidth, then you provide the bandwidth.
The alternate is content distribution monopolies based around ISP's. Want to distribute direct forget it, they will put you out of business by slowing down or even blocking your traffic, you will be required to use a digital publisher (see same middle men arseholes back again).
As for VOIP as long as it is all on their network fine, off their network cripple the traffic to make competing ISPs look bad, do it intermittently to get away with it. How about intercepting all traffic and I mean all traffic including VOIP to inserts ads. How about monitoring and data mining all traffic to sell the data to marketing firms. How about monitoring all traffic and cutting of any complaint traffic.
There is a substantial deference between, hey wikileaks how do I submit these documents and hey wikileaks how do i obtain these documents. In point of fact in the first case the documents are unsubstantiated documents until they are submitted and of course once published they can not be defined as factual by anyone else the 'oops' US government.

Perhaps you maybe just might of heard of it, the government is the people, the people are the government (how can you steal what you in part already own, the theft comes when certain individuals try to substantively increase the size of their part). This of course ceases to be true when private for profit interests corrupt the government to suit their own psychopathic greed, this same group will also tell the rest of the public the government is bad and they are better off having nothing to do with it and ignoring it, the simplifies control of the government by those individuals.
PS the easiest way to end corporate welfare is to limit the size of corporations and reduce the risks associated with their limited liability, no corporate welfare is required as the loss of any particular corporation is of no great significance.
The Republicans are already talking about de-funding the FCC, basically running them into bankruptcy if they try to enforce net neutrality. So the big miss was getting it into law, now it is just a matter of politicians bankrupting a government agency if the agency attempt to uphold the law against the wishes of corporate interests.
Besides let's work the the car analogy and the opposite of net neutrality, net economic freedom. If you can pay more for your drivers licence and registration why shouldn't you be able to drive faster. So the average drivers licence and registration pays for the average speed, so the current base fee for drivers licence and rego should only get you base speed, say 20 mph in the slow lane and you always have to give way to higher licensed and registered traffic.
You could even have an entry level where you can only drive to subsidised location, like your employer (if they are willing to pay) or the supermarket (if they pay), every where else you need to catch a specially licensed taxi but the basic access still gains you access to the footpath (excluding pedestrian crossings you have to pay to use those after all you are obstructing a full fee paying driver). As for the rich, why not open slather access, when it comes to crashes it is never their fault and they always get access. As for public transport that's a socialist plot, if you can only afford minimum you should be made to walk.
You should easily be able to see the substantial opportunities for profit in the scheme, especially when drivers licences and registration are privatised and, where would the Ferengi 'er' Conservatives be without profit.
