No, Gemini and Mercury were the programs that taught NASA that they needed a requirements based approach instead of agile.
Yes it was, in conjunction with IBM under Bill Tindall according to NASA history Howard W. "Bill" Tindall started with Mercury and Gemini ground software and later made a significant contribution to the quality of the Apollo on-board software. It wasn't called "Agile" back then however the contribution of quality were Agile components like iterations and time boxes. That's why I referred to "Agile" with it's collective name - capital "A" instead of "agile".
We learned from killing a few people and blowing up a bunch of rockets that we needed a better design approach.
So point to the specific incidents where a software problem killed people in NASA's space programs.

I continue to hear this and see absolutely no evidence of it. I see evidence to the contrary, in the US, India and Europe, over the last 20 years. Generally, it's RPM/RH that is first listed. It's not alphabetical. This isn't because they are lucky. The simple explanation is that RH is the most frequently used and therefore put at the top as a simple matter of UI layout (most common choices go to the top of a list, within reason).

P.P.S => For the "trolls" Big O is a measure of complexity, whereby the length of time, measured in cycles, of a looped operation is compared to the amount of data being processed. For example, a hash lookup is O(1), a search through a list is O(N) (because the number of operations is proportional, on average, to the number of items), and a bubblesort is O(n!) (I think, I can't remember, it's been a while since I did this.) The reason it's relevant to a hosts file generator is that if you use the wrong algorithms, you'll find that the length of time it takes to process your initial hosts list will increase exponentially, or worse, as you add more hosts.

He probably wrote it in good faith, but the reality is that it's a product that can be used easily and effectively to hide spyware on a computer. So many anti-malware organizations have flagged it as majorly dangerous.

Well, if you're not a spyware maker then obviously it's unfortunate, but in the US libel does require an element of malice, and from some quick Googling, it does appear that multiple sources believe you are, indeed, a spyware author. They may or may not be correct, but someone can legitimately, in good faith, say you are one because it's very easy to believe you are.
I say this because I googled "Alexander Peter Kowalski". The very first link is this one:

Now, in the above posting, you threaten the author of a spyware removal tool because his tool (and apparently others too) have identified one of your programs as being spyware. As with Jeremy Reimer and, indeed, Clone here, you were, apparently quick to yell "Libel", and your responses appear (to me - and I'm just giving you my honest opinion, this is how you're coming across to me) as hysterical and nutty. The impression anyone reading the article gets is not "My, Mr Kowalski has been given a very rough time, poor thing.", but "Some Spyware manufacturer is trying to intimidate anti-spyware people."
And, for what it's worth, doing some googling on the executable described by the above link continues the theme whereby apparently rather a lot of people are under the impression your software is spyware. The first eight links I get when I Google are all from anti-spyware organizations, and the other two on the first page aren't flattering either.
Coupled with the fact that respected agencies such as CA are apparently standing behind their claim your software is spyware, there is nothing libelous about Clone's viewpoint. It might be wrong, I can't comment, I've never been privileged enough to have your software on my PC (I'm a GNU/Linux guy personally), but it's not a malicious written falsehood that damages your reputation.
So, no, Clone isn't libeling anyone here. Here's what to do about it if you're telling the truth:
- Take a step back. Honestly, take a deep breath, and stop getting angry.
- Determine why CA and others believe your software to be spyware. If there's an innocent explanation, give it. If there isn't, well, the difference between a spyware author who harasses those who point it out, and one who keeps quiet, is that more people hear about the allegations against the stalker.
- Be the man. You've upset a number of people, and a quick Google reveals it to be more than Jeremy Reimer and Clone over here. If your software is being tagged as spyware then it rather suggests that intentionally or not, you've also upset a number of people who use your software. Fess up, fix the issues, and take some steps to mend fences between you and your critics.
I'm saying this not because I think you will, but because it's what you have to do. At this point, Alexander Peter Kowalski's name is mud. It's mud not so much because someone got confused and thought you were making spyware when really you were making the coolest app evah, but because you haven't handled the criticism of the app particularly well.

And, yes, my apps' still there (zero threat levels though - it CAN be "misused" though, this happens to the "best of us", see below in fact)

They list it - with ZERO threat levels... EVEN THOUGH I PASSED EVERY ONE OF THEIR 21 QUESTIONS FOR REMOVAL!
(It happens - Just as they do to others' wares, such as Nir Sofer's wares (he has this happen ALL THE TIME, wrongfully, because his apps are good, & just like PING could issue a "ping of death"? His apps may be a 'double-edged sword' is all - same thing's happened to a former "colleague" of mine (mid 1990's for Sunbelt Software in Dr. Mark Russinovich of Microsoft, when he & I both made wares for them back then)).
In any event?? Have some balls, answer the question, that's all!

Oh, really? Care to show me a legal precedent OR ruling of that being the case, were I switching IP addresses to overcome the "10 posts per 24 hour limit" on AC users? I sign off as APK, so I am NOT "misrepresenting myself" here, in case you hadn't noticed, lol!
(Now, on "changing IP addy"? Uhm, who says I am doing THAT anyhow to beat the restriction - and so what IF I did? I could just as easily say I turned off my modem & PC, & upon restarting it?? I have a new IP address... pretty simple - I'd like to see you backup that b.s., with legal evidence to what you said, because, anyone can turn off their modem & upon restarting be assigned a new IP address, assuming they don't have a STATIC IP!)
Produce some US legal documentation of what you just said... thanks! I, for one, would like to see it - because it appaernty wouldn't account for folks that keep their power off when not using a system... so, show us your LEGAL proof of what you stated now above, ok?
