I agree, I was hoping for a list of quality games. Here's some of my favorites for GBA and DS:
GBA:
- Blast Arena Advance
- Elements Of Darkness
- Clem the Retarded Elf
- Anguna (disclaimer: I wrote this one)
DS:
- Johnny Platform's Biscuit Romp
- Twin Isles
- A touch of war
- Video games hero(guitar hero clone)
- AngunaDS (again, disclaimer, I wrote this)
Now I just need to see if I can find my list of quality homebrew NES and Dreamcast games....

Make the interface not suck
If you're going for a 'change as little as possible' angle, try and improve the interface of some part of the game. Give it a decent level select so people can try different parts whenever. (If in doubt, look up Perfect Dark's level select rules.)
That's basically the story behind the 'two D-pad scrolling method', level select, 'shoulder pause', 'frame advance' and zoom features in Lemmings DS.
Make it so that people can focus on the game rather than battling against the keyboard for what they need to do. (Remember the DOS version of D/GENERATION?) -

If you have a not yet softmodded Xbox, and an original copy of Splinter Cell, The Gentlemen's Complete And Unabridged Guide For The Softmodding Of The Microsoft Xbox will help you download all the necessary files to softmod the Xbox, copy them over, install the softmod and install XBMC for Xbox.

If you have a DS and a way to play homebrew Lemmings DS is an excellent port. Not only is it a great bathroom game using a stylus is so much nicer than a mouse.

Generator sites
I think 'generator sites' and stuff like YTMND or Objection! would be useful to investigate because there's a big cross section of users and influences all coming together to create wild and varied (well... maybe just wild then) content.

I agree with what you are saying though. I don't buy flash carts or mod chips to play homebrew stuff. I might be buying a PSP very soon now as well.
