If a medical practice and it's customers want to participate in the government system, they can do so and pay a fee to support it. Or, they can choose not to participate and they don't have to pay.
Make any government health care program optional. If you want to be a member, you can pay and be subject to the program's rules and regulations. If you don't want to be a member, you don't have to pay and aren't subject to the rules and regs.
If I am thinking of opening a business in California or another state, all other factors being equal, would I open it in a state where I can train employees in advanced methods only to have them bolt to a competitor and enrich them? No, I would choose the other state.
Sell public schools to the highest bidder (returning the proceeds to the local residents), and drastically cut local taxes so parents keep more of their money. The market would provide incredibly innovative and inexpensive education, just like any unregulated market. For example, if you had told people 3, 10, 20 years ago that with a few clicks of a mouse you could have a complete dual core computer delivered to your door in a few days for $350, they'd think it was a miracle. Just imagine what entrepenurial innovators would deliver for education.
So the do-gooders pass a law that criminalizes sending e-mail, wreck a guy's life, and 3 people end up dead. Congratulations to everyone who supported this spam law.

I have Verizon Wireless, and if I connect to my voicemail, I have the option to record a message and have it sent to a phone number that I enter. I've used it from time to time. Not sure if I can send to a non-VZW number though.
