The above statement is based on my wife's treatment in the USA and in the UK.
In my own case, I had a problem for which one of the treatments is surgery. I was referred to a hand specialist who only discussed the surgical option with me. When I asked about the alternative treatment that I had discovered using Google, his response was that he didn't do that treatment and I would have to see another doctor. Had I not researched it for my self, I would never have known that there was an alternative. That's on top of the fact that I had to pay for a completely useless consultation with the hand surgeon.
Summary, even if you have good insurance in the USA, you may not be getting the best treatment.

How many times have I read that the reason for the very high food prices is that the theaters don't get keep revenue from the ticket sales? Is this not true? Have we been lied to and the reason the food prices are so high is simply that people will pay the absurd prices?

The back end servers remained running *BSD for a long time afterwards. When they did eventually transition, it required compiled binaries (instead of php or perl or whatever was used in the original iteration of Hotmail) and much newer, more powerful servers to achieve the same performance.
IMO, the tickets should be paid, at least the first ticket per person. I think that's the only ethical course of action.

If I am on the other side of the trade and I made a profit, why should I lose this just because they claimed that there was an error? As an ordinary small trader, I can't get trades reversed because I made a mistake.
The big advantage of buying direct from the airline is what happens if you need to change the flight. You deal with the airline directly instead of some (probably incompetent) third party.

In the UK, the M25 (circular road around London) has a variable speed limit.
As traffic becomes heavier, the speed limit drops. But all lanes get the same speed limit. The limit is heavily enforced with cameras.
With the limit set quite low, the traffic proceeds much more uniformly, there is no advantage to changing lanes, everyone drives at the limit.
The result is that more cars can flow past any point on the road. It's another example of the effect predicted by the paper.

This is presumably a profitable business for the USPS, an increase in prices would just drive the business elsewhere.
What they want is the end of the USPS, with private services replacing it. Removing a profitable business from the USPS furthers this aim.

I think that, as long as the biomass comes from new-growth forests, then one can consider it carbon-neutral.
