Is it easy to get out of one of these things? I can imagine somebody getting in one, picking a destination, being told that they're 'suspicious' (or the thing just breaking down) and being trapped inside one.

If you're going for a 'change as little as possible' angle, try and improve the interface of some part of the game. Give it a decent level select so people can try different parts whenever. (If in doubt, look up Perfect Dark's level select rules.)
That's basically the story behind the 'two D-pad scrolling method', level select, 'shoulder pause', 'frame advance' and zoom features in Lemmings DS.
Make it so that people can focus on the game rather than battling against the keyboard for what they need to do. (Remember the DOS version of D/GENERATION?)
Metal Gear Solid, Snake never looks out of the screen at you and engages you. I can think of plenty of games where the character you play does. Just like there's a fair few movies where a character breaks the fourth wall.

Has anybody tried running FUSA on a S&L PSP with CFW? If that works as it's described, you should already be able play anything using the composite video cable.
