The ideal solution would be to redesign the road system so that autonomous vehicles could happily work however that would require trillions of dollars. so these cars are going to have iterative updates to learn and cope with all the situations even humans have to learn different road rules where I live we drive on the left driving in the U.S. meant swapping everything over even after 6 weeks still managed to get on the wrong side once. This is compounded by differences in different areas for various reasons even humans have trouble with these but I am sure an AI once they teach/program it will do much better. WRT the death of the pedestrian the driver should not have been on her phone as her car was at best an version 0.1 at his stage and her job was to watch the car and help teach it any new situation much like a learner driver and their instructor.
Its a combination of things. e.g. There is a job that I need done regularly a manual labour job in this instance, we used to get 4-5 guys and get it done in 1-2 days then 15 years back OHS demands meant the job started to take 2-3 days this was bearable we could charge a little more.
The government mandated various thing like workers and public liability insurance this could be dealt with too it affected everyone and could be shown to clients and the better employers had it anyway, actually levelled the playing field a bit. Then the last 3-4 years the workers got lazy spent to much time on their phones so the job started taking longer and longer on top of the extra costs impact from distracted workers on insurance etc.. So I made a piece of very low tech less than $500 that modified the main tool used on the job. A robot was available for $130,000. The job is now done by 1 person in 2 days. I no longer hire those workers and cut insurance costs by 80% as its based on number of workers and wages paid.
So I like said a combination of things. All it takes is one last push for that the necessity to go over the edge

The same goes for many businesses the costs to start now are much higher not just in tech and add to that ongoing costs e.g. energy cost that are increasing I see a lot of younger people give up once they realise that business they want to start-up would require 50k just to contemplate its not just big companies that make it difficult government and legislative requirements crush lots of starry eyed would be business owners
