In my opinion, this is another case than the Hot Coffee one.
Whereas the Hot Coffee minigame was a mod, that couldn't have been unlocked in-game by any cheatcodes otherwise, the Sims 2 game has the command "intprop censorgridsize" which can be turned off : And thus enabling nudity in an even lower rated game than GTA was.

The next GTA seems to be going the MMORPG-way though (from early reports) : Then again, if they keep churning out sequels to GTA as good as they did until now, I won't lose hope in them releasing another great, singleplayer-focused, GTA game.
The problem with what you are describing (not saying there isn't a solution) would be that alot of the good gameplay has to do with the several hot-spots around the map. Those spots are carefully planned and tested, and when you would have the ability to take key-spots down/remove or create key entries towards a point, it would hurt the flow of the map, and thus the gameplay.
The Red Faction series have an almost unlimited ammount of freedom concerning your surroundings, but it still has its boundaries too.
If you haven't played these games yet, you can probably get them out of the bargain bin and I would definitely recommend it (at that price;)) as it offers a few hours of fun, and a unique experience blowing up everything;)
Not -all- people type without looking at their keyboard : Besides that, when using various graphical tools (Photoshop, 3DSMax), you don't have your hands constantly on the keyboard (mouse/digital pen) : So there's where it comes in use too.
This keyboard will be great for mapping keys for games
