I think this essentially what you're seeing. Someone who has the time and inclination to figure out any UI that comes across their path will never understand people who have different priorities. Clearly, everyone else is an idiot.

Not really. Unlike the UK, almost all doctors in America are private practice doctors and not on government salary. The same with hospitals, a mix of private and local/state public hospitals. The health care reform legislation passed is mainly for insurance; the government won't change its control of doctors or which private plans people choose. So the government really isn't in charge of health care, although they've taken a more regulatory role in insurance.

Improving infrastructure made sense when the American empire was in an ascendant period. Now that it's on the wane the most profitable course of action is to skim as much cream as we can until it collapses.
Square-Enix is just not in touch with what makes a game good as a game. They have a knack for compelling stories, and they have a solid art-design team, but these aren't enough to make a good game.
I think every one of SE's games has come with a moment when I wondered if any of them actually play video games.

You get to build out on private property without needing to pay market value for the land, OR negotiate with every person whose land you cross.
It's a bit of a red herring, though. There's no good reason for infrastructure to be privately held. "If you build it, they will come" should be the motto of every municipality.
